Cedar Crest is looking for a qualified Director of Social Services to join our family. We are a resident- and family-focused community that values team building and leadership to support residents and their families.

The Director will oversee social services, assist with admissions and referrals, and help shape a positive facility culture. If you are driven and team-oriented, this is an ideal opportunity to lead a compassionate social services program and contribute to a supportive environment

Formación

• Bachelor's Degree in Social Work and either an LSW or MSW from an approved school.
• Minimum of one year experience.
• Demonstrated ability to assess resident needs, develop and implement a plan of care, and coordinate discharge planning.
• Knowledge of accreditation standards and compliance requirements.
• Member in good standing in the National Association of Social Workers.
• Qualified, compassionate, and dedicated to a job well done.

Responsabilidades

• Keep abreast of federal and state regulations and professional standards; propose policy updates to the Executive Director/LNHA.
• Implement policies for identifying medically related social and emotional needs of residents.
• Participate in community planning related to facility and resident/family needs.
• Participate in discharge planning; develop and implement social care plans and resident assessments.
• Provide consultation to staff and community agencies to develop social service programs.
• Review and develop corrective plans for social service deficiencies noted during surveys; provide written reports.
