The Pennant Group, Inc. is seeking a compassionate clinical leader to oversee all nursing and resident care services at our Stoughton, WI senior living location. An active LPN license is strongly preferred, with 1–3 years in assisted living or healthcare.

Qualifications

Current LPN license preferred.
Minimum 1-3 years of experience in assisted living, senior living, or healthcare.
Prior leadership or supervisory experience strongly preferred.
CPR certification as required.

Responsabilités

Lead and oversee all clinical operations and resident care services.
Conduct resident assessments, service planning, and ongoing evaluations.
Ensure compliance with state regulations, resident rights, HIPAA, and infection control standards.
Recruit, coach, mentor, and develop a high-performing clinical team.
Partner with residents, families, physicians, pharmacies, and therapy providers to coordinate care.
Monitor quality outcomes, medication management, clinical documentation, and risk reduction initiatives.
Support occupancy efforts through resident assessments and admission evaluations.
Foster a culture of accountability, compassion, and excellence in resident care.
