Job summary

Country Arch Care Center seeks a reliable LPN to provide direct nursing care and supervise nursing assistants on the 3pm–11pm shift. Responsibilities include leading shift activities, charting, medication administration, and coordinating with physicians and departments to ensure quality resident care.

Requirements include an active NJ LPN/RN license, NJ Board of Nursing licensure, CPR, and prior long-term care supervisory experience. Strong English communication skills are essential.

Qualifications

• Licensed Practical Nurse with current unencumbered state license.
• Must read, write, speak and understand English.
• CPR certification required.
• Experience in Long Term Care preferred.

Responsibilities

• Direct day-to-day functions of nursing assistants.
• Plan shift services with assigned staff for the shift.
• Provide written and oral reports on shift activities.
• Admit, transfer and discharge residents; lead unit staff.
• Complete and file required records on admission/transfer/discharge.
• Receive and transcribe physician orders and document.
• Chart nursing notes reflecting care and resident response.
• Submit accident/incident reports to Director.
• Perform routine charting per policy and procedures.
• Prepare and administer medications as ordered.
• Maintain rapport with inter-departmental staff to ensure care needs.
• Create warm, calm unit atmosphere; maintain shift morale.
• Report nursing status at shift handover; rounds with physicians.
• Deliver services such as catheterization, tube feedings, dressings, etc.
• Monitor staff safety with equipment and supplies.
• Maintain resident care confidentiality and privacy.
• Report any unauthorized disclosure of information.
• Perform other duties as assigned.
