Avamere Skilled Advisors, LLC d/b/a Avamere Living at Avamere at Bethany in Portland, OR is seeking a Resident Care Coordinator to coordinate resident care needs and staffing, maintain schedules, and organize clinical records. The role supports the Director of Health Services and ensures regulatory compliance.

The RCC trains Caregivers and Medication Techs, handles daily living assistance, conducts audits, and participates in QA meetings. On-call coverage after hours and weekends is required.

Qualifications

• High school diploma or equivalent required.
• Must maintain CPR/First Aid certification.
• Ability to obtain a Food Handler's Card.
• Experience in electronic medical records documentation.
• Prefer a licensed Certified Nursing Assistant in the state where the position is held.
• Must be 18 years or older.
• Friendly, attentive, and professional demeanor with residents and guests.
• Compassion for and desire to work with the elderly.

Responsibilities

• Coordinate resident care needs and staffing, including scheduling.
• Maintain physician appointment and pharmacy logs; organize clinical records.
• Provide orientation and on-the-job training to Caregivers and Medication Techs.
• Assist with daily living activities (bathing, dressing, eating).
• Conduct audits on a weekly, monthly, and quarterly basis.
• Complete quarterly evaluations of residents as assigned.
• Participate in Quality Assurance meetings and staff development.
• Manage scheduling of Caregiver and Medication Tech staff; ensure adequate coverage.
• Support caregivers in daily resident care, communication, and problem-solving.
• Assist with new resident assessments, move-ins, and transitions.
• Ensure supplies and equipment meet day-to-day needs.
