Qualifications
- Social Service Coordinator Certificate within three months of hire.
- CA Certified Nursing Assistant license within six months of hire.
- CPR Certification preferred.
Responsibilities
- Assess residents' social service needs at admission and quarterly thereafter.
- Develop and carry out individualized psychosocial care plans.
- Complete psychosocial sections of the MDS form timely with care plans.
- Inform all departments of upcoming appointments and needs.
- Interview and assess each patient after admission to service.
