• Provide counseling and one-on-one support to residents and families.
• Coordinate admissions, discharges and room transfers.
• Complete and support MDS and care planning processes.
• Act as liaison with community agencies and external resources.
• Support Resident Council and coordinate transportation as needed.
• Ensure compliance with social services regulations and Sunrise standards.
• Manage departmental operations, staffing, scheduling and engagement initiatives.
• Partner with leadership on budget management and financial oversight.
