Summary

The Resident Care Coordinator (RCC) shall provide for the needs of residents, resident care associates, and the resident care department. The RCC will assist with the overall clinical/wellness operation and work with the Resident Care Director (RCD) to optimize the efficiency and effectiveness of the resident care department.

Essential Functions, Duties, and Responsibilities
  • Participate in and complete community's assigned in-service training requirements, whether in person, written, or online.
  • Monitor changes in resident needs, preferences, and health status; report these changes promptly to the RCD, RN, or designee.
  • Prepare and manage the work schedule for Resident Care staff, including RCAs, Medication Aides, Nurses, etc., ensuring it is posted at least 2 months in advance and managing attendance.
  • Review daily time clock punches, hours per resident day, overtime, and agency use; report findings to RCD/ED; correct missed punches as needed.
  • Order clinical and incontinence supplies.
  • Provide orientation and training to new Resident Care team members on electronic health records and medication administration records.
  • Monitor and review RCA training logs; assist in creating monthly training calendars; report compliance to RCD, BOD, and ED.
  • Document all services provided, incidents, behaviors, and residents' refusal of services; input data into the community's tracking system.
  • May conduct or facilitate programming that offers cognitive stimulation, socialization, physical activity, and spiritual connection.

Qualifications

Job Qualifications:

  • Qualifications may vary by state, including certifications, experience, and training.
  • Knowledge, Skills, and Abilities:
    • Computer proficiency for documenting services.
    • Fluency in English (oral and written).
    • Organizational skills and proficiency in Microsoft Office.
    • Ability to learn new software platforms (e.g., Yardi, OnShift).

Education: Completion of CNA training or equivalent experience.

Experience:

  • Prior direct care experience preferred.
  • Experience caring for seniors with dementia preferred.
  • Experience in nursing/wellness operations for senior housing preferred.

Certificates/Licenses: Current license/certification preferred as per state regulations; provide documentation to the RCD or designee.

Physical Demands:

  • Dexterity, visual and auditory acuity, ability to lift at least 15 lbs, push/pull carts, walk, sit, stand, crouch, kneel, reach, and use good body mechanics.
  • Ability to assist residents weighing approximately 150 lbs and push an occupied wheelchair up to 200 feet.
  • Maintain a positive attitude and be aware of potential exposure to blood, chemicals, and infectious materials.
