Job summary

The ClearView Group - Solutions for Health is seeking a compassionate nurse to provide direct resident care in a safe, supportive environment. You will assess conditions, monitor changes, and coordinate care with physicians, families, and the facility team to maintain quality outcomes.

Responsibilities include administering medications, updating care plans, and training new Household Assistants. The role requires strong communication and bedside skills to promote resident wellbeing.

Qualifications

• Experience working with elderly or disabled population preferred.
• Experience or training in behavior management or rehabilitative principles.
• Previous lead role experience.
• CPR/First Aid certification upon hire or obtainable after hire.

Responsibilities

• Assesses resident conditions; monitors for changes and responds with appropriate action.
• Communicates resident needs or condition changes to physician, family and staff.
• Reviews resident care plans; completes documentation.
• Coordinates with other staff and participates in care plan staffing and discharge planning.
• Performs treatments including dressing changes and catheter care as needed.
• Reviews and administers medications; verifies orders and communicates with pharmacy as needed.
• Documents nursing assessments, interventions and responses; reports to physician and family as appropriate.
• Provides direct patient care including repositioning and hygiene; monitors skin needs.
