Description

Resident Care Manager (RN)

Setting: Skilled Nursing

Status: Full-Time

Location: Salem Transitional Care - 3445 Boone Rd. SE Salem, OR 97317

Responsibilities:

  • Develop and review care plans and treatments to ensure they align with residents' needs and wishes
  • Coordinate and revise the MDS, including implementing CAA’s and Triggers
  • Conduct daily rounds to monitor nursing staff performance
  • Review nursing notes for accuracy and completeness
  • Maintain good communication with facility staff to meet residents' needs
  • Participate in discharge planning and report preparation for the Care Plan Committee
  • Delegate, train, evaluate, and support nursing staff (RN, LPN, CNA)
  • Provide direct nursing care as needed
  • Assist the Director of Nursing and fill in as required
  • Participate in facility surveys and compliance reviews
  • Recommend updates to procedures, policies, and practices to ensure quality care
  • Attend ongoing education and lead in-service training for staff

Qualifications:

  • Degree in Nursing from an accredited institution
  • Active, unencumbered RN license in Oregon
  • At least 1 year of supervisory experience in healthcare
  • Training in rehabilitative and restorative nursing
  • Knowledge of nursing practices, laws, and regulations
  • Experience with Electronic Medical Records and documentation systems
  • Confidentiality compliant with HIPAA
