Job summary

Avamere Court at Keizer seeks an experienced Resident Care Manager LPN to plan, coordinate resident care and review treatments in line with each resident's care plan. The role supports the Director of Health Services and steps in to supervise and deliver care when needed.

You will train new care staff, conduct audits, ensure documentation, and maintain compliant operations. The position is full-time at Avamere Court at Keizer with hours including on-call after hours and weekends.

Qualifications

  • Nursing degree from an accredited college or university.
  • Current, unencumbered, active license to practice as an LPN in this state.
  • 1+ years of experience as a supervisor in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices.
  • Knowledge of nursing and medical practices and regulations for nursing care communities.
  • Must maintain and hold an active CPR/BLS during employment.
  • Experience with electronic medical records and computer documentation systems.
  • Passion for caregiving and serving our senior resident community

Responsibilities

  • Document information pertaining to changes in residents' status, needs, and appointments.
  • Ensure proper documentation of all care plans, nurses' notes, and all other resident correspondence.
  • Provide orientation and on-the-job training to Caregivers and Medication Techs.
  • Conduct weekly, monthly, and quarterly audits.
  • Participate in evaluations and service plan reviews as assigned.
  • Participate in facility quality improvement plans.
  • Provide direct nursing care as necessary.
  • Assist the director of health services and fill in as needed.
  • Make daily rounds to ensure all personnel are performing their work assignments.
  • Participate in facility surveys by authorized government agencies.
  • Complete quarterly evaluations of residents as assigned by the Executive Director.
  • Participate in Quality Assurance Meetings.
  • Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy.
  • Complete ongoing training as assigned.
  • Complete other duties as assigned.
  • Support caregivers in daily resident care, communication, and problem-solving.
  • Assist with new resident assessments, move-ins, and transitions.
  • Ensure adequate supplies and equipment are on hand to meet day-to-day needs.
  • Participate in staff development efforts.
  • Maintain a clean and organized environment.
  • Follow established healthcare standards and practices.
  • Ensure the safety and comfort of residents.
  • Communicate effectively with residents, family members, and staff.
  • On-call staffing responsibilities after hours and weekends as needed.

Job Summary

The Resident Care Manager LPN is responsible for planning, developing and coordinating the patient care for each resident and reviewing the resident's medical and nursing treatments to ensure they are in accordance with the resident's care plan. The Resident Care Manager LPN assists the Director of Health Services in training the new care staff. When the Director of Health Services is absent, the Resident Care Manager LPN is responsible for the supervision and delivery of resident care services in compliance with Avamere' s policies and state and federal regulations.

Essential Duties and Job Responsibilities
