Job summary

Avamere Court at Keizer in Keizer, Oregon, is seeking a Resident Care Manager - Licensed Practical Nurse (LPN) to oversee resident care planning and implementation. The LPN will supervise care staff, review treatments, and ensure compliance with Avamere policies and regulations.

The role includes training new staff, maintaining quality of care, and stepping in for the Director of Health Services as needed. Strong leadership and clinical skills are essential.

Qualifications

  • Nursing degree from an accredited college or university.
  • Current, active LPN license in Oregon.
  • 1+ years of supervision experience in healthcare.
  • CPR/BLS certification required.
  • Experience with electronic medical records (EMR) and documentation.
  • Knowledge of nursing practices and regulations governing care communities.
  • Passion for caregiving and serving seniors.

Responsibilities

  • Document changes in residents' status, needs, and appointments.
  • Ensure care plans and nurses' notes are properly documented.
  • Provide orientation and on-the-job training to Caregivers and Medication Techs.
  • Conduct weekly, monthly, and quarterly audits.
  • Participate in evaluations and service plan reviews.
  • Provide direct nursing care as necessary.
  • Support the director of health services and fill in as needed.
  • Maintain a warm, patient, enthusiastic, and calm environment.

Resident Care Manager - Licensed Practical Nurse

Status: Full Time

Location: Avamere Court at Keizer

Job Summary

The Resident Care Manager LPN is responsible for planning, developing and coordinating the patient care for each resident and reviewing the resident's medical and nursing treatments to ensure they are in accordance with the resident's care plan. The Resident Care Manager LPN assists the Director of Health Services in training the new care staff. When the Director of Health Services is absent, the Resident Care Manager LPN is responsible for the supervision and delivery of resident care services in compliance with Avamere's policies and state and federal regulations.

Essential Duties and Job Responsibilities
  • Document information pertaining to changes in residents' status, needs, and appointments.
  • Ensure proper documentation of all care plans, nurses' notes, and all other resident correspondence.
  • Provide orientation and on-the-job training to Caregivers and Medication Techs.
  • Conduct weekly, monthly, and quarterly audits.
  • Participate in evaluations and service plan reviews as assigned.
  • Participate in facility quality improvement plans.
  • Provide direct nursing care as necessary.
  • Assist the director of health services and fill in as needed.
  • Make daily rounds to ensure all personnel are performing their work assignments.
  • Participate in facility surveys by authorized government agencies.
  • Complete quarterly evaluations of residents as assigned by my Executive Director.
  • Participate and provide feedback in Quality Assurance Meetings.
  • Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy.
  • Complete ongoing training as assigned.
  • Complete other duties as assigned.
  • Support caregivers in daily resident care, communication, and problem-solving.
  • Assist with new resident assessments, move-ins, and transitions.
  • Ensure that adequate supplies and equipment are on hand to meet the day-to-day operational needs of the facility and residents.
  • Participate in training and support staff development efforts.
  • Maintain a clean and organized environment.
  • Follow established healthcare standards and practices.
  • Ensure the safety and comfort of residents.
  • Communicate effectively with residents, family members, and staff.
  • On-call staffing responsibilities after hours and weekends as needed.
Requirements:

Requirements and Qualifications

  • Nursing degree from an accredited college or university.
  • Current, unencumbered, active license to practice as an LPN in this state.
  • 1+ years of experience as a supervisor in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices.
  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care communities.
  • Must maintain and hold an active CPR/BLS during employment
  • Experience with electronic medical records and computer documentation systems.
  • Passion for caregiving and serving our senior resident community
