Job summary

Responsibilities

• Assess psychosocial status of patients and share findings with the RN.
• Perform social evaluations and plan person-centered interventions.
• Maintain clinical documentation for all patients referred to social work.
• Provide information and coordinate referrals for residents and families regarding needs.
• Inform residents or families about community resources and services available.
• Act as liaison between residents, families and community agencies.
• Collaborate with facility staff to support resident care.
• Participate in total care planning and case conferences for residents referred to social services.
• Assist physicians and team in understanding social and emotional factors affecting health.
• Establish behavior monitoring and interventions to reduce medication use.
• Address resident behaviors to prevent harm to self or others.
• Participate in discharge planning and advise on post-discharge practical needs.
• Coordinate medication review to determine gradual dose reduction.
