Monarch Communities in Toms River, NJ is seeking a Care Manager to assist residents with daily living activities in accordance with care plans. This full-time, hourly position requires 1 year of resident care experience and a valid CNA/HHA or Direct Care competency where applicable, with strong English communication skills.

Qualifications

• High School Diploma or GED; at least 1 year resident care experience.
• Certified Nursing Assistant or Certified Home Health Aide or Direct Care Staff Competency exam (PA ONLY).
• Knowledge and experience in working with the elderly population, preferred dementia experience.
• Able to speak, read and write in English.

Responsabilités

• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan.
• Respects the needs of team members and residents by adhering to schedule and documenting attendance properly.
• Maintain the safety, comfort, dignity, and confidentiality of all residents.
• Encourages and escorts residents to meals and activities when needed.
• Ensures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts.
• Document all services provided in EHR for your assigned residents during your shift.
• Monitors and reports any change of condition in a resident immediately to your supervisor.
• Keeps electronic documentation devices safe and secure within the community; documents care/services during the shift.
• Participates in and attends all in-service trainings as scheduled.
• Ensures compliance with all Federal, State, and local regulations, as well as community policies and procedures.
• Provides basic housekeeping and/or laundry services when needed.
• Other duties as assigned.
