The position requires travel between sites and may include after-hours on-call duties.

• Coordinate and schedule medical, psychiatric, dental/vision appointments and liaise with physicians and health professionals regarding residents' medical needs.
• Track all physical and health-related appointments (medical, dental, vision).
• Prepare and complete documentation per program standards within required time frames.
• Perform Direct Support Services when needed.
• Use computer to perform duties and related administrative tasks.
• Support after-hours on-call system as part of duties.

Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors offices , health care providers, laboratories along with communicating and liaison with physicians , nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident. ( 65%)2. Track all physical, health care ,dental , vision appointment . (10%)3. Prepare and complete all necessary documentation in accordance with program standards within required time frames as required. ( ie. MA 51) (10%)4. Perform Direct Support Services when needed (10%).5. Work on computer to perform the above duties - ongoing6. Part of the after-hours on call system- OngoingThese are not all encompassed job responsibilities.
