Job summary

Kennedy Care Center is seeking a dedicated RN/Qualified caregiver to coordinate care plans for residents. You will communicate with residents and families about plans of care, participate in team stand-ups, and ensure timely documentation and adherence to regulatory requirements.

The role emphasizes independent work, strong time management, and collaboration with physicians, staff, and payers to deliver safe, cost-effective care in a high-volume setting.

Qualifications

  • Bachelor's Degree in Nursing or Social Work.
  • RN license preferred; LVN/LPN acceptable.
  • Two (2) year clinical nursing experience preferred.
  • Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
  • PCC Knowledge (if applicable).
  • Ability to read and interpret policy and procedure manuals; ability to present information clearly.

Responsibilities

  • Communicate regularly with residents and families about Plans of Care and treatment protocols.
  • Attend morning meetings to facilitate communication with the team.
  • Monitor and adjust resident statuses based on changing needs.
  • Complete documentation in a timely manner.
  • Coordinate admissions, discharges, and referrals with the facility and third-party payers.
  • Maintain accurate charting and paperwork; exercise sound judgment with physicians and staff.
