We are currently seeking an experienced and motivated Resident Care Coordinator (LPN) to join our growing team.

The Resident Care Coordinator plays a key leadership role in supporting the daily clinical operations of the facility and helping ensure our residents receive exceptional, compassionate care. This position works closely with nursing leadership, frontline caregivers, residents, and families to promote strong communication, consistent care delivery, and a positive resident experience.

The ideal candidate is a Licensed Practical Nurse (LPN) who is organized, dependable, approachable, and passionate about supporting both residents and the nursing team. Job Responsibilities and Duties:
