Job summary

Hamilton Health Care Center is seeking a Licensed Practical Nurse (LPN) to join our growing nursing team on a PRN basis. You will conduct assessments, admit/discharge residents, provide wound care, supervise CNAs and CMTs, and respond to emergencies.

This role offers flexible shifts and a supportive, patient-centered work environment. We require LPN licensure and a compassionate, professional approach to care.

Qualifications

• Must hold valid LPN licensure in Ohio.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
