Prestige Senior Living – Five Rivers in Tillamook, Oregon is seeking an Interim/PRN Resident Care Coordinator to ensure resident care needs are met under Health Services leadership. You will coordinate care, assist in interviewing and training health services staff, and help maintain safe, well-staffed operations in a home-like assisted living setting.

You will also be involved in medication management, coordinating resident changes, and ongoing safety education.

Qualifikationen

• At least one year as Personal Care Attendant or 6 months as Medication Technician/Aide, or prior Resident Care Coordinator experience.
• Experience in an Assisted Living setting preferred; ability to work on-call, weekends and holidays as scheduled.
• High school diploma, GED or equivalent knowledge and skills obtained through a combination of education, training and experience.

• Partner with health services leadership in developing and implementing Cooperative Service Plans and ensure plans are updated in a timely manner.
• Coordinate and assist in interviewing, hiring, onboarding, and training health services staff.
• Collaborate with leadership to create and maintain health services schedules accommodating time off while maintaining staffing levels per resident care needs and state requirements.
• Maintain medication carts and rooms in an organized, clean, and safe manner; submit orders for medications, supplies and equipment as prescribed.
• Coordinate information on changes in residents’ condition, needs, appointments, and outside services; communicate to appropriate individuals.
• Work as a Medication Aide and/or Personal Care Attendant as needed to cover shifts and stay apprised of resident care concerns.
• Educate, monitor and practice safety procedures relating to equipment, chemicals, tools and utensils including infection control and universal precautions.

In this job, everyday you will…

Education & Experienceyou need to qualify:

• You will need at least one-year experience as aPersonal Care Attendant or 6 months as a Medication Technician/Aide, orprevious experience as a Resident Care Coordinator.
