Job summary

Avista Senior Living in Sun City West, AZ seeks a proactive Resident Care Coordinator to lead the health services team and ensure compassionate, personalized care for residents. You will supervise staff, coordinate schedules, and liaise with home health and hospice providers.

The ideal candidate holds an AZ caregiver license with 3–5 years of healthcare experience and can train new staff. This role requires strong communication skills and flexibility to work weekends and evenings.

Responsibilities

• Fill gaps in CNA/Med Tech schedule and cover regular shifts as needed.

• Maintain MAR and communicate regarding medications; manage medication cart.
• Coordinate with Home Health and Hospice providers.
• Organize monthly In-service training.
• Maintain shift change notes and communicate with administration.
• Fulfill other duties as assigned by administration.
