Job summary

Spring Gardens of Heber seeks a dedicated Resident Care Coordinator to join our health services team in Heber City, UT. You will lead with compassion, ensuring high-quality care for elderly residents and coordinating with CNA/Med Tech staff.

The ideal candidate holds a current CNA license, has experience in assisted living or nursing homes, and is available weekends with a 8:30-5:30 schedule. You will contribute to a supportive, engaging work environment and help uphold our standards of

Qualifications

• Must have a current CNA license.
• Experience caring for elderly residents in assisted living or nursing home settings.
• Weekend availability required; able to work 8:30-5:30 Mon-Fri and fill in as needed.

Responsibilities

• Fill gaps in CNA/Med Tech schedule as needed.
• Maintain CNA/Med Tech schedule.
• Assist with new hire training as assigned by administration.
• Maintain MAR and communicate about medications; manage medication cart.
• Coordinate with Home Health and Hospice providers.
• Organize monthly in-service trainings.
• Maintain shift change notes and communicate with administration.
• Fulfill other duties as assigned by administration.

We are currently looking to hire a passionate and service- minded leader to serve as our Resident Care Coordinator and become a key member of our health services team! Our ideal applicant will have a CNA license and have weekend availability to work Monday-Friday 8:30-5:30pm and be willing to fill in for shifts as needed.

• Coordinating with Home Health and Hospice Companies.
• Organizing monthly In-service training.
• Maintaining shift change notes.
• Communicating regularly with administration.
• Fulfilling other duties as assigned by administration.
