Job description

JOB SUMMARY:

A Resident Care Coordinator will work closely with the Health Services team in our Community Based Care facility to ensure the delivery of quality care to all residents. This position is key in organizing and assisting with training new care staff and providing ongoing mentorship to current employees.

RESPONSIBILITIES:

• Coordinate onboarding and orientation for new employees
• Serve as the lead trainer for the care team (including Caregivers and Med Techs)
• Oversee ongoing training for all staff
• Review assignments with the Executive Director and Director of Health Services
• Assist residents with activities of daily living, encouraging self-care and independence
• Support leadership in creating staff schedules
• Document any changes in resident condition
• Be willing to cover shifts when needed
• Perform other duties as assigned
