Job summary

Ivy Living is seeking a Resident Care Coordinator in California to support resident health needs and coordinate care services. The role collaborates with physicians, nurses, and coordinators to ensure smooth operations and compliant assessments.

The ideal candidate will have supervision experience, strong MS Office skills, and flexibility to cover holidays and varied shifts while maintaining a compassionate, resident-centered approach.

Qualifications

• Must be at least 18 years old.
• High school diploma or equivalent required.
• Prefer 1 year of supervision/management experience.
• Ability to train staff and adjust schedules as needed.
• Proficient with MS Office applications (Word, Excel, PowerPoint).
• Criminal background and health screenings including PCR test prior to start.

Responsibilities

• Communicate with healthcare professionals about resident care needs.
• Assist Health Services Director with coordinating medical services and resident escorts.
• Participate in resident assessment program per policy and regulations.
• Recruit, develop, manage and schedule care providers and meds techs.
• Maintain a safe environment following safety standards.
• Foster teamwork across departments.

Position Resident Care Coordinator

Shifts: Sunday through Thursday, including holidays.

Pay Range: 23 per hour 25 per hour

Responsibilities

• Communicate with community healthcare professionals including physicians discharge planners nurses and state agencies on the care needs of residents as needed
• Assist the Health Services Director in coordinating incidental medical dental vision hearing and podiatry care for residents and arrange resident escorts s needed
• Participate in the communitys resident assessment program in accordance with company policy and state regulations
• Recruit develop manage and schedule Care Providers and Medication Technicians
• Maintain a safe and secure environment for all staff residents and guests following established safety standards
• Encourage teamwork through cooperative interactions with co workers and other departments

Qualifications

• Must be at least eighteen 18 years of age
• High school diploma or equivalent
• Prefer one 1 year of experience supervising and managing employees
• Ability to effectively train staff
• Flexibility in work schedule as evidenced by working holidays evenings weekends and additional shifts when necessary to ensure adequate coverage within the department
• Ability to operate a personal computer and be proficient in MS Office programs Word Excel Publisher PowerPoint etc
• Must pass a Criminal Background check and Health Screening tests including COVID 19 Polymerase Chain Reaction PCR test within 72 hours of start date
