Qualifications
- CNA license certification required prior to hire.
- Must have current CPR certification and fingerprint clearance.
- Minimum 6 months CNA/LNA experience preferred.
- Recent LTC/SNF or Transitional Rehab experience preferred.
Responsibilities
- Assist guests/patients during admissions, transfers, and discharges, ensuring personal care needs align with the care plan.
- Review care plans daily to determine changes in the daily care routine as per the plan.
