Job summary

Great Lakes Management is seeking a Resident Assistant for the PM shift in Sun Prairie, WI. This part-time role offers the 2pm–10pm schedule with a quarterly two-week cycle and a pay range of $18–$22 per hour, based on experience and CBRF certification.

Qualifications

• Six months of experience in long term care or assisted living (preferred).
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (preferred).
• Ability to make responsible choices and act in the resident’s best interests.

Responsibilities

• Assist residents with ADLs as indicated in the individualized service agreement.
• Help residents in dining room and participate in activities and outings.
• Communicate effectively with residents, families, and notify the case manager of changes in condition.
• Complete required daily documentation, service schedule, and related records.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
