Great Lakes Management is seeking a Part-time Resident Assistant in Sun Prairie, WI. The role supports residents with daily living activities, offers emotional support, and assists in dining and activities under the resident service plan.

The ideal candidate will have at least six months of experience in long term care, preferably with Nursing Assistant or CNA certification, and a strong commitment to resident wellbeing.

Qualifications

• Six months of experience in long term care or assisted living (preferred).
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (preferred).
• Ability to make responsible choices and act in the resident’s best interests.

Responsabilités

• Assist residents with ADLs as indicated in the service plan.
• Help residents reach and maintain independence.
• Provide residents with physical and emotional support.
• Assist with treatments as delegated by the RN per the service plan.
• Observe clients and report changes to the case manager.
• Interact with residents to provide reassurance and engagement.
• Complete required documentation daily and in the care book.

• Hours – PM Shift (2pm-10pm). In a set 2 week schedule - Week 1: Monday, Wednesday, Saturday. Week 2: Sunday, Tuesday, Thursday.
• Pay Range – $18-$22 per hour depending on CBRF certification and experience.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Qualifications :

• Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant (Preferred)
