EverTrue is seeking a weekend program Personal Care Assistant to provide supportive and functional care to residents under nursing leadership. The role collaborates with an interdisciplinary team to maintain residents’ independence and dignity.

Responsibilities include assisting with daily living activities, observations, documentation, and responding to resident needs while adhering to safety and policy standards.

Qualifications

• Previous resident/patient care experience preferred.
• Ability to read simple instructions, short correspondence, and memos; write simple correspondence.
• Basic computer skills for data entry and retrieval.

Responsibilities

• Assist residents with activities of daily living and personal care.
• Follow standards of practice and EverTrue policies and procedures.
• Maintain safe, clean resident care environment and respond to requests promptly.
• Document observations and vital signs as required.
• Act as resident advocate and participate in care planning with the team.

Supports residents in attaining and/or maintaining their highest practicable level of functioning by providing and/or assisting residents with personal care and performance of all routine activities of daily living2. Provides care and services in accordance to current standards of practice as defined by training standards and EverTrue Clinical Policies and Procedures3. Maintains the appropriate resident care environment and responds to resident requests for assistance in a timely manner4. Follows all safety procedures and protocol as dictated by EverTrue policy, federal and state regulations and standard nursing practice and reports all hazardous conditions, defective equipment and accidents or incidents to their nursing leader immediately5. Performs and provides proper resident observations and record keeping; Functions as the resident’s advocate, participates in care planning and communicates to their nursing leader and interdisciplinary team any changes observed6. May be instructed to assist and escort residents to/from dining room and in-house activities; monitor residents in the dining room at mealtimes7. May be assigned the responsibility of documenting residents’ vital signs including blood pressure, temperature respiration, weight checks, and other pertinent information8.
