Job summary

Great Lakes Management is seeking a part-time Resident Assistant in Bloomington, MN to provide hands-on care and support to residents as outlined in service agreements. You will assist with ADLs, support daily activities, and communicate with residents, families, and the case manager as needed.

Responsibilities include helping residents maintain independence, assisting with treatments as delegated by the RN, documenting daily care, and engaging residents in activities.

Qualifications

• Six months of experience in long term care or assisted living preferred.
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant preferred.
• Ability to make responsible choices in residents’ best interests.

Responsibilities

• Assist residents with ADLs and help maintain independence.
• Support residents in dining and activities; communicate needs to the case manager.
• Provide physical and emotional support as delegated by the RN.
• Document daily care and observations consistently.
• Monitor changes in condition and report them promptly.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
