Job summary

Great Lakes Management is seeking a Resident Assistant in Maplewood, MN to support residents with daily living activities and personal care on the 2pm-10pm shift. The role emphasizes compassionate, person-centered care and effective communication with residents, families, and the care team.

You will assist with ADLs, monitor conditions, and document care activities daily, while collaborating with the nursing team to ensure safety and quality of life for residents in a long-term care setting.

Qualifications

• Six months of experience in long-term care or assisted living (Preferred).
• Registered Nursing Assistant or CNA certification preferred.
• Ability to make sound decisions in residents' best interests.

Responsibilities

• Assist residents with ADLs per service agreement.
• Provide physical and emotional support to residents.
• Assist with treatments as delegated by the RN per the resident service agreement.
• Document daily care and communicate changes to the case manager.
• Engage residents in activities and maintain safety.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Qualifications :

• Six months of experience in long term care or assisted living residence (Preferred)
• Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant (Preferred)
• Ability to make responsible choices and decisions and act in the resident’s best interests.
