Job summary

Great Lakes Management is seeking a dedicated Resident Assistant to provide hands-on care and emotional support to residents in Maplewood, MN. You will assist with ADLs, participate in dining and activities, and maintain clear documentation per the individualized service plan.

Six months of long-term care experience and CNA certification are preferred; shift differentials and a 401(k) match are offered. Position hours are 2pm–10pm, on-site at 1910 County Rd C E, Maplewood.

Qualifications

• Six months of experience in long term care or assisted living residence.
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (Preferred).
• Ability to make responsible choices and decisions and act in the resident's best interests.

Responsibilities

• Assist residents with ADL's (Activities of Daily Living) as indicated on the individualized service agreement
• Help each resident to reach and maintain the highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistant observes clients and reports changes in physical and emotional condition such as change in attitude, loss of appetite, weight, etc. to the case manager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands-on care, both physical and emotional, as outlined in each resident's service agreement. Responsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assists in the dining room and participating in activities and outings.
