Qualifications

• Bachelor’s degree in social work or related field from an accredited institution.
• One year of supervised social work experience in health care.
• Experience in casework is required.
• Valid driver’s license and clean driving record.
• Desire to work with the elderly and cooperate with staff.
• English language literacy at an intermediate level.

Responsibilities

• Welcome and support residents and families during admission.
• Assess residents' psychosocial, emotional, and support needs.
• Participate in care planning and interdisciplinary team meetings.
• Assist with discharge planning and transitions of care.
• Document admission and discharge forms and SCREEN when applicable.
• Provide counseling and support to residents and families.
• Advocate for resident rights, dignity, and quality of life.
• Collaborate with nursing, therapy, and clinical teams to support resident needs.
