Job summary

Qualifications

• Bachelor's degree in Social Work or a related Human Services field.
• Current Social Worker license.
• Ability to organize and prioritize many tasks effectively.
• Skilled communicator, director and motivator; respond to emergencies when required.

Responsibilities

• Meet with administration and staff to plan social services.
• Advocate for residents' rights and coordinate with resources.
• Complete Social Service History and MDS sections as scheduled.
• Educate residents on Advance Directives and end-of-life planning.
• Coordinate referrals to ancillary services and transportation.
• Document interactions and discharge planning.
