Atrium Center for Rehabilitation and Nursing in Brooklyn is seeking an experienced Social Worker to join our compassionate team. The role focuses on resident advocacy, discharge planning, and coordinating services to support residents and families.

The position offers a full-time schedule, competitive salary of $60,000–$65,000, and benefits including 401(k), PTO, and health coverage. MSW preferred; bachelor’s in a related field with at least one year in healthcare social work.

Qualifications

• MSW preferred.
• Bachelor’s degree in a related field.
• Minimum one year of supervised social work experience in healthcare.
• SNF experience strongly preferred.
• Strong interpersonal and communication skills.
• Compassionate, organized, and dedicated to improving residents’ quality of life.

Responsibilities

• Conduct resident and family interviews to develop social histories and care plans.
• Partner with families to create programs supporting residents’ emotional, social, and psychological well-being.
• Assist with discharge planning, financial resources, and coordination of community services.
• Collaborate with nursing, administration, and medical staff to ensure holistic care.
• Advocate for residents by addressing social and emotional needs with clear communication.
