Job summary

Sapphire Center for Rehabilitation and Nursing, a premier skilled nursing facility in Queens County, seeks a full-time Social Worker (MSW) to support residents’ emotional and social well-being. The ideal candidate is compassionate, professional, and experienced in the social services field within long-term care.

The Social Worker will plan and direct social services, ensure compliance with federal/state standards, and assist residents and families with admissions, discharges, and service

Qualifications

• MSW or Master’s Degree in a related field.
• Licensed and credentialed as a social worker by the NY Department of Education.
• Experience in human services or social work serving adults as required by state regulations.
• Basic computer knowledge, especially Microsoft Word.

Responsibilities

• Plan, develop, implement, evaluate, and direct social service programs.
• Coordinate Individualized Service Plans (ISPs) and ALR data.
• Discharge planning and arranging community/homecare setup.
• Collaborate with management team to coordinate resident services.
• Document case management notes.
