Cura of Le Sueur is seeking a compassionate Social Services Coordinator to advocate for residents and guide families through transitions in skilled nursing and long-term care. You will build relationships, coordinate services, and collaborate with an interdisciplinary team to ensure resident rights and needs are met.

The role emphasizes resident-centered support, communication, and problem-solving, with involvement in care planning, discharge coordination, and community resource connections.

Formación

• Background in social work, human services, psychology, sociology, healthcare, or a related field.
• Experience in skilled nursing, long-term care, healthcare, case management, or social services is preferred.
• Strong communication and relationship-building skills.
• Ability to remain professional and solution-focused during challenging situations.
• Strong organization and follow-through.
• Ability to manage multiple priorities throughout the day.
• Comfortable working both independently and as part of an interdisciplinary team.
• Understanding of resident rights, community resources, and long-term care regulations is a plus.

Responsabilidades

• Build strong, trusting relationships with residents and their families.
• Advocate for resident needs, preferences, choices, and rights.
• Assist new residents and families as they transition into the facility.
• Complete psychosocial assessments and required social services documentation.
• Participate in care conferences and collaborate with the interdisciplinary team to develop care plans.
• Coordinate discharge planning and connect residents with community resources, services, and transportation.
• Maintain regular communication with residents and families and assist with questions or concerns.
• Support residents experiencing emotional, behavioral, social, or adjustment-related challenges.
• Coordinate behavioral health and other supportive services when needed.
• Assist with advance directives and other resident planning needs.
• Help resolve resident and family concerns and ensure follow-up.
• Collaborate with nursing, therapy, MDS, activities, dietary, providers, and facility leadership.
• Support compliance with resident rights and state/federal requirements.
• Participate in QAPI, quality improvement, and survey readiness activities.
• Help create an environment where residents feel heard, respected, safe, and supported.
