Qualifications

• Bachelor’s degree in Social Work or related human services field.
• Strong leadership, communication, organizational and problem-solving skills.
• Ability to work with residents, families, staff and regulatory agencies.
• Knowledge of resident rights, care planning and discharge planning.

Responsibilities

• Lead, develop, and oversee the Social Services Department to ensure residents receive appropriate medically related social services.
• Complete and oversee social assessments and contribute to individualized care plans.
• Advocate for residents’ rights, dignity, preferences, and psychosocial well-being.
• Coordinate admissions, discharge planning, and transitions to community-based services.
• Assist residents and families with accessing financial assistance and community resources.
• Oversee advance care planning and communication of residents’ wishes.
• Address grievances, document resolutions, and ensure follow-up.
• Ensure compliance with state and federal regulations and facility policies.
• Collaborate with nursing, administration, and interdisciplinary teams to improve resident outcomes.
• Support quality assurance initiatives and continuous improvement.
• Report and follow-up on allegations of abuse, neglect, or exploitation.
