Qualifications

• High school diploma or equivalent is required. Preferable one-year experience in a long term care facility.

Responsibilities

• Participate in planning and conducting individual, small and large group activities.
• Assist in providing good communication between employees, residents, their families, and the public to meet residents' needs.
• Develop monthly activity calendar and maintain attendance records.
• Assist in discharge planning and resident assessments.
• Arrange transportation for residents to appointments or outings.
