Job summary

Pioneer Trace Healthcare & Rehabilitation is seeking an Activity Assistant to support the Activities Director with coordinating and providing an activities program tailored to each resident’s needs, preferences and strengths.

The role includes leading group and individual activities, drafting monthly calendars, coordinating with volunteers and families, and assisting with the activity plan of care while documenting participation and maintaining privacy and safety standards.

Qualifications

• High school graduate.
• One year experience preferred, preferably in a long-term care setting.
• Proficiency in standard English.
• Able to perform assigned duties with minimal oversight.

Responsibilities

• Provide group and individual activities for residents according to identified needs and preferences.
• Offer activities within and outside the facility.
• Draft and post monthly activity calendars.
• Coordinate with volunteers, families and community agencies.
• Contribute to development and revision of the activity plan of care as directed.
• Document activity participation and responses in the medical record.
• Assist with coordinating Activities Department services with other departments to ensure quality care.
• Maintain privacy of records and confidential information.
• Follow established safety and infection control procedures.
• Participate in required inservice training.
• Maintain safe working conditions and dress code compliance.
• Report complaints and grievances to supervisor.
• Adhere to and promote the Code of Conduct.
