Job summary

Responsibilities

• Plan and conduct individual, small and large group activities.
• Facilitate good communication between staff, residents and families.
• Assist in developing monthly activity calendars and attendance records.
• Support discharge planning and resident assessments related to activities.
• Arrange transportation for residents for outings or appointments.
• Create a monthly activity schedule including outings and in-room activities.
• Assist with activity-related documentation and progress notes.
• Keep the Activity Department clean, organized and secure.
