The role emphasizes warm, empathetic interactions, collaboration with the care team, and adherence to facility policies. Candidates should have a high school diploma or equivalent and strong interpersonal skills.

Qualifications

• High school diploma or equivalent required.
• Experience organizing and facilitating activities in a skilled nursing facility or with geriatric populations preferred.
• Knowledge of dementia care is a plus.
• Strong interpersonal and communication skills.

Responsibilities

• Organize, implement and direct scheduled activities of a recreational, physical, educational, social and spiritual nature for residents both in groups and for individuals.
• Ensure participation of all residents and that activities offered can be applied to various levels of functioning according to recorded resident assessments.
• Perform room visitations and activities as required by schedule and/or observed resident need.
• Keep clear and concise documentation on logs and in resident charts of resident participation in activities as required by state regulations and facility policy.
• Monitor and supervise residents at all times during activities to ensure resident safety.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
