Job summary

Hagerstown Nursing and Rehab, part of the CommuniCare Family, is seeking an Activities Leader to deliver individualized activity care for residents. You will lead one-to-one and group activities on/off units, coordinate events, and assist with scheduling, documentation, and community involvement.

You will support residents through diverse activities, advocate for their needs, and collaborate with the activity coordinator and other departments to ensure quality care and meaningful engagement.

Qualifications

• High school diploma or GED required.
• Healthcare setting experience preferred.
• Experience with related software applications preferred.
• Ability to make independent decisions when warranted.
• Tactful communication with team, residents, families and staff.
• Basic computer literacy.
• Active state driver's license required.
• CDL license may be required to drive large vehicle.

Responsibilities

• Lead a variety of activities for residents on/off assigned units and in the community.
• Provide supplies and equipment for residents to participate in activities.
• Plan, develop, coordinate and evaluate resident-centered activities.
• Assist in planning holidays, events, and community outings.
• Escort residents to activities and maintain schedules and announcements.
• Maintain up-to-date activity documentation for residents.
• Coordinate with other departments for activity logistics.
