Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communication between staff, residents, families and the public.
• Participate in community planning related to resident services.
• Assist in developing monthly activity calendar and attendance records.
• Support QA/Assessment in action plans to correct quality deficiencies.
• Assist discharge planning and activity care plans for residents.
• Arrange transportation for residents to outings, appointments, and discharge.
• Develop monthly activity schedules including outings and in-room activities.
