Job summary

Diversity Group is seeking an Advanced Practice Nurse - Certified Nurse Practitioner to serve as Clinical Faculty in a VA Primary Care Service Residency program. You will provide supervision, guidance, and mentoring to residents while overseeing their ongoing competency and progress.

You will manage a panel of patients with acute and chronic conditions, emphasizing prevention and evidence-based care, and collaborate with the Primary Care Team to optimize outcomes.

Responsibilities

• Clinical faculty provides supervision, guidance, and mentoring to residents.
• Conducts structured evaluations of residents' competency and progress.
• Oversees residents' continuity of care under supervision.
• Regular meetings with the program director to assess progress and development.
• Mentors residents and other faculty as needed.
• Manages a panel of patients with acute and chronic conditions focusing on prevention.
• Develops individualized plans for population health and disease management.
• Leads collaboration with Primary Care Team and interdisciplinary staff.
• Orders and interprets labs and diagnostic studies; prescribes interventions.
• Initiates referrals to specialty services and educates patients and families.
