Diversity Group is seeking an Advanced Practice Nurse - Certified Nurse Practitioner to serve as Clinical Faculty in a VA Primary Care Service Residency program. You will provide supervision, guidance, and mentoring to residents while overseeing their ongoing competency and progress.

You will manage a panel of patients with acute and chronic conditions, emphasizing prevention and evidence-based care, and collaborate with the Primary Care Team to optimize outcomes.

• Mentors residents and other faculty as needed.

• Develops individualized plans for population health and disease management.
• Leads collaboration with Primary Care Team and interdisciplinary staff.
• Orders and interprets labs and diagnostic studies; prescribes interventions.
• Initiates referrals to specialty services and educates patients and families.

Summary

The Advanced Practice Nurse - Certified Nurse Practitioner (NP) practices in Primary Care Service Residency Program Clinical Faculty to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice - consultation - and management.

Duties

• Major duties include - but are not limited to: The primary clinical faculty is assigned to the residents for the entire year and provides clinical supervision - guidance - and mentoring
• The clinical faculty will be responsible for structured and formal evaluations of the residents' competency - performance - and progress
• The resident's continuity of care clinic is under the supervision and oversight of their clinical faculty member
• The residency program director will meet regularly with the clinical faculty to evaluate progress and ongoing professional development needs
• The clinical faculty will be available to the residents as needed for mentorship and guidance
• Residents - clinical faculty - and the program director will meet weekly as a group to provide support and guidance
• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
