Job summary

Concerto Renal Services is seeking a PRN Renal Dietitian to provide nutrition education, counseling, and interventions for ESRD patients in clinical settings. You will assess nutritional needs, design care plans, and monitor progress in collaboration with the interdisciplinary team.

Travel to assigned facilities and coverage as needed while staying current with CMS regulations and company standards. This role offers flexible, PRN hours suited to dialysis care environments.

Responsibilities

• Nutrition assessments and care plan development for ESRD patients.
• Medical nutrition therapy and monitoring based on labs and dialysis adequacy.
• Tailored patient education and counseling considering individual backgrounds.
• Collaborate with IDT and travel to facilities to provide coverage as needed.
• Maintain credentials and ensure CMS compliance with standards.
