Devoted Health is seeking a Nurse Practitioner/Physician Assistant leader to oversee the Transitions of Care (TOC) program and drive clinical excellence across the APP team.

The role combines direct patient care (30-day post-discharge follow-ups) with 60% leadership duties, including KPI development, performance management, recruitment, onboarding, and policy creation within the TOC program.

Qualifications

• 5+ years in clinical practice as an Advanced Practice Provider (NP/PA).
• At least 1 year of seeing post-discharge patients as an APP.
• 2+ years of leadership or management experience.
• Active RN and APRN licenses; national APRN certification; multi-state licensing ability.

Responsibilities

• Provide TOC patient care 40% of the time, including post-discharge follow-ups and gap filling.
• Lead and manage TOC APPs, aligning with KPIs and program targets.
• Develop processes, policies and recruit/onboard TOC APPs with education and training teams.
• Collaborate with Medical Director and TOC leadership to optimize workflows.
