UnitedHealth Group's OptumCare is seeking a Telephonic Nurse Practitioner for a remote, part-time role licensed in Colorado. The position includes after-hours call responsibilities, with a strong emphasis on care coordination and telehealth services across multiple markets.

The role supports 24/7 coverage, requires 40 hours per week including evenings, nights, holidays and every other weekend as needed, and offers a $10,000 sign-on bonus for external candidates.

Qualifications

• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted license in the state of Colorado and ability to start the process of obtaining additional licensure prior to start date.
• Board certification in Family/Adult/Gerontology NP.
• 3+ years of clinical APC experience.
• Active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Proficient computer skills including EMR documentation.

Responsibilities

• Provide telephonic/telehealth care to members, coordinating with caregivers, facilities and providers.
• Utilize EMR to deliver acute care across all shifts and holiday hours.
• Deliver cost-effective, quality care and manage chronic and acute conditions in collaboration with physicians.
• Coordinate care as patients transition between care settings and adjust plans as needs change.
• Maintain patient privacy and comply with CMS documentation requirements.
