Job summary

Optum Home & Community Care is seeking a National On-Call Advanced Practice Clinician to deliver telephonic/telehealth care 24/7 from a remote location. The role supports patients across settings, coordinating with facilities, providers and caregivers to ensure timely, cost-effective care.

The successful NP will hold an active CT or RI license, board certification, and have strong EMR skills, with a focus on comprehensive assessments and documentation compliant with CMS regulations.

Qualifications

• NP license in CT or RI and eligibility for licensure in both states if applicable.
• Graduate from an accredited NP program and board certified.
• Experience with telehealth platforms and remote patient care coordination.

Responsibilities

• Provide telephonic/telehealth care and direction to patients 24/7 including holidays.
• Utilize EMR to document acute and chronic care assessments and plans.
• Coordinate care across caregivers, facilities, hospitals and providers.
• Attend mandatory trainings and engage in quality improvement activities.
