Qualifications

• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted license in the state of Colorado and ability to start the process of obtaining additional licensure prior to start date.
• Active Nurse Practitioner certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with Family/Adult/Gerontology options.

Responsibilities

• Provide telephonic/telehealth care and direction to patients, caregivers and facility staff for 24/7 coverage.
• Utilize EMR to provide acute care and coordinate care across care settings.
• Deliver cost-effective, quality care and collaborate with physicians or specialists to manage chronic and acute conditions.
