Job summary

Telos Health Systems is seeking a Telemedicine & Behavioral Health Provider for skilled nursing and post-acute care in North Carolina. This part-time, fully remote role combines primary care and psychiatric-mental health support via telemedicine, with day and night shifts and flexible scheduling.

Ideal candidates hold NP/PA/PMHNP licenses (NC/KY/PA preferred) and are comfortable independent in a protocol-driven telemedicine environment, collaborating with nursing teams, and providing escalation

Qualifications

• Active licenses in at least NC, KY, and PA; willingness to obtain additional state licenses as telemedicine expands.
• Dual certification in primary care and psychiatric-mental health is strongly preferred.
• Experience in telemedicine settings with elderly/post-acute populations.
• Ability to work independently within established protocols and guidelines.

Responsibilities

• Respond to urgent and acute calls from SNFs.
• Complete scheduled medical and behavioral health visits and follow-ups.
• Evaluate and manage SNF medical concerns via telemedicine.
• Assess mood, anxiety, sleep, cognition, and behavior.
• Provide treatment plans and escalation guidance as needed.
• Collaborate with nursing staff and telemedicine leadership.
• Document encounters accurately in the EHR.
