Job summary

Agape Behavioral Health is seeking a Telehealth Therapist for our South Florida facilities. The role conducts psychosocial assessments, develops Master Treatment Plans with the Treatment Team, coordinates discharge planning, and maintains communication with families and referral sources.

Remote work is available with a Florida license required (LMHC, LCSW, LMFT). The position requires a Master’s Degree, active Florida licensure, and at least 1 year of substance abuse or mental health treatment

Qualifications

• Master's degree required.
• Active Florida license: LMHC, LCSW, or LMFT.
• At least 1 year experience in substance abuse or mental health treatment preferred.

Responsibilities

• Review assessments from referring facilities and note problems requiring further attention.
• Contact referral source and other key individuals to gather information and maintain contact throughout patient’s treatment.
• Gather diagnostic information via psychosocial assessment and develop an Integrated Summary for treatment planning.
• Develop a comprehensive problem list and present psychosocial and diagnostic information to the treatment team.
• Review initial treatment plan in first session and ensure staff assignments and responsibilities are understood.
• Formulate a comprehensive treatment plan with measurable goals and timeframes.
• Participate in multidisciplinary treatment plan reviews with preparation and understanding of patient issues.
• Develop interventions and state the planned frequency of interventions.
• Maintain clinical records according to policies and ensure documentation is timely and secure.
• Continue treatment planning throughout stay and revise plans as needed.
• Lead and co-facilitate group therapy sessions to foster self-disclosure and group skills.
• Complete activity and group plans for the Clinical Director.
